During rhinoplasty surgery, Dr. Bull separates the skin of the nose from the supporting framework of bone and cartilage below. He then reshapes the nasal framework and re-drapes the skin over it. For most patients getting a reduction in the size or an alteration of the shape of their nose, or having a hump removed, Dr. Bull makes the incisions inside the nose. This allows him to move the nasal bones closer in order to narrow the nasal bridge. He can also reduce the nasal tip by re-sizing the cartilage through the inside incision, though this may also require making a small incision at the base of the nose.
The angle between the nose and the upper lip can be improved by elevating and trimming the septum, the dividing wall between the two chambers of the nose. It is also sometimes necessary to narrow the base of the nose. This procedure involves removal of skin and tissue from the inside of the nose, so the scars are usually not noticeable. Note that rhinoplasty performed to correct congenital deformities, relieve breathing problems, or repair defects caused by trauma may be covered by insurance. Such cases are judged on an individual basis and require pre-authorization.

Nasal pads may be inserted at the time of a rhinoplasty surgery to protect the septum. These pads are removed the day after the procedure. A lightweight splint is also applied after surgery to maintain the new shape of the nose. The splint is usually removed within a week. Although most nose surgery patients are able to get up and move around later the same day of their procedure, strenuous exercises—particularly activities that might elevate blood pressure—should be avoided for at least four weeks. Rhinoplasty patients should also avoid sun exposure for about eight weeks, since post-surgical skin is highly sensitive. Exposure to ultraviolet radiation can cause scars to grow darker and more apparent.
The nose takes some time to settle into its new shape after rhinoplasty surgery. While the initial changes—from a straighter and flatter bridge to a refined tip to a generally smaller appearance—will be noticeable just after the procedure is completed, the nose’s ultimate look may not be achieved for a year due to the very gradual process of swelling reduction in the area.
